Cosmetic dermatology has become a hot topic with many exciting procedures that continue to showcase skills and artistry of cosmetic surgeons. The concept of fractional energy-based devices has become popular in recent years. Primarily they are much safer than the traditional energy-based devices. This is true for non-ablative fractional lasers, ablative fractional lasers, and the radiofrequency (RF) fractional devices as well. The RF fractional devices can be divided into two main types—pin-based fractional RF and needle-based fractional RF.The advantages of fractional RF in general are that RF energy is color-blind. This is very important because we live in a skin of color world and we all worry about the impact of postinflammatory hyperpigmentation, in patients treated with energy-based devices. With fractional RF devices, the risk of PIH in our patients is greatly reduced and is why these devices have become so popular worldwide. What is needed now is further clinical work to delineate what makes the devices different.
